RESUMO

Objective:To analyze the clinical characteristics of patients with double-positive anti-myelin oligodendrocyte glycoprotein (MOG) antibody and anti-N-methyl-D-aspartate receptor (NMDAR) antibody, so as to raise awareness of such diseases and improve the prognosis.Methods:Eighteen patients (double positive group) with positive serum anti-MOG antibody and cerebrospinal fluid anti-NMDAR antibody in Huashan Hospital, Fudan University from March 2017 to March 2020 were retrospectively analyzed. Using the SPSS software for simple random sampling, anti-MOG group(20 cases) and anti-NMDAR group (20 cases) were randomly selected at the same time for comparison. The anti-MOG group referred to the patients only with positive serum anti-MOG antibody. While the anti-NMDAR group referred to the patients whose cerebrospinal fluid anti-NMDAR antibody was positive. The clinical characteristics, laboratory examination results, radiological characteristics and prognosis of the three groups were collected and analyzed.Results:There was no statistically significant difference in demographic data among the three groups ( P>0.05). The symptoms of patients in the double-positive group were divided into two categories by cluster analysis, which corresponded to the symptom groups obtained by cluster analysis of the anti-MOG group and the anti-NMDAR group, and the same result was verified by correspondence analysis. Compared with the anti-MOG group, the incidence of epilepsy (10/18 vs 3/20, P=0.016), psychosis and behavior change (8/18 vs 0/20, P=0.001), visual disturbances (8/18 vs 17/20, P=0.016), dysarthria/dysphagia (8/18 vs 1/20, P=0.007) was significantly different in the double-positive group ( P<0.017). Compared with the anti-NMDAR group, the incidence of ataxia (8/18 vs 19/20, P=0.001), psychosis and behavior change (12/18 vs 1/20, P<0.001) was significantly different in the double-positive group. There was no statistically significant difference in the combination rate of thyroid peroxidase antibody, thyroglobulin antibody and antinuclear antibody between two groups, and the cerebrospinal fluid pressure, white blood cell count, protein, glucose, chloride and positive rate of oligoclonal band were also not statistically different between two groups ( P>0.017; P<0.017 indicates statistically significant difference by Bonferroni corrected multiple comparisons). Compared with the anti-NMDAR group, whether the brain magnetic resonance imaging had lesions was different in double positive group (18/18 vs 8/20, P<0.001). The initial modified Rankin Scale (mRS) scores before treatment were different among the double positive group, anti-MOG group and anti-NMDAR group (3.72±0.96, 2.75±0.97, 3.95±0.76, respectively, F=10.004, P<0.001), but there was no statistically significant difference in the scores after six-month treatment (1.22±1.44, 0.40±0.75, 1.20±1.24, respectively, F=3.153, P=0.051), and the recurrence rate of the disease was different among the three groups (8/18, 14/20, 5/20, respectively, χ2=10.004, P=0.017). Conclusions:Anti-MOG antibodies and anti-NMDAR antibodies could exist at the same time, showing clinical phenotype overlap, which was a new syndrome called the overlapping syndrome of myelin oligodendrocyte glycoprotein antibody-associated disease and NMDAR encephalitis, MNOS. The condition of MNOS patients was more severe than that of patients with MOG antibody-associated disease (MOGAD), but patients with MNOS, MOGAD, and anti-NMDAR encephalitis all responded well to immunosuppressive therapy. It was suggested that early second-line immunotherapy should be given to reduce the recurrence rate in patients with MNOS and MOGAD.

Objective To firstly report the clinical features,diagnosis and treatment response of patients with anti-γ-aminobutyric acid type A receptor (GABAAR) encephalitis in China,thus raising neurologists' awareness of this emerging type of autoimmune encephalitis.Methods Specific anti-GABAAR autoantibodies in the serum and cerebrospinal fluid (CSF) of patients with suspected autoimmune encephalitis but negative for commercial available antibody tests were detected by live cell-based assay (CBA).The clinical features,laboratory examinations and treatment of two cases of autoimmune encephalitis with anti-GABAAR autoantibodies were analyzed,who admitted to Huashan Hospital,Fudan University between 2013 and 2014.Results By using live CBA,serum and CSF of the two patients diagnosed with possible autoimmune encephalitis both contained autoantibodies targeted to the GABAAR.These two patients had onset symptom of seizure or refractory seizures.Memory impairment,psychiatric symptoms and decreased consciousness were also presented.One patient was combined with mass in anterior superior mediastinum.Both patients had multifocal cortical and subcortical T2 /fluid attenuated inversion recovery-weighted images hyperintensity signal on brain magnetic resonance imaging.The two patients had poor response to antiepileptic drugs,but showed noticeable recovery with sufficient immunotherapeutic treatments.Conclusions Anti-GABAAR encephalitis is characterized by prominent epilepsy and multifocal abnormalities on brain magnetic resonance imaging.Autoantibodies specifically against GABAAR could be detected by CBA in this group of patients.Early diagnosis and immunotherapy are critical to improve clinical symptoms and outcomes of the disease.

Objective To study the effect of psychological intervention combined with the analysis of Budesonide and Formoterol Fumarate Powder for Inhalation in treatment of chronic obstructive pulmonary disease. Methods 100 patients with chronic obstructive pulmonary disease treated in our hospital from February 2015 to March 2016 were selected and randomly divided into the control group and the experimental group, with 50 patients in each group. The control group was treated with the Budesonide and Formoterol Fumarate Powder for Inhalation treatment,the experimental group on the basis of psychological intervention on the mental status of patients and patients,strengthen communication and exchanges, increase confidence in the treatment and the treatment compliance of patients. Results After treatment, the SGRQ score of the experimental group was (57.33±16.81), and the score of the control group was (43.86±12.68) points, with statistical difference (P<0.05). The scores of depression and anxiety in the experimental group were significantly better than those in the control group,with statistical difference(P<0.05). Conclusion Budesonide and Formoterol Fumarate Powder for Inhalation combined with mental intervention in the treatment of chronic obstructive pulmonary disease can significantly improve the patients, improve the quality of life, help patients recover as soon as possible, has clinical significance.

AIM:To investigate the role of reactive oxygen species (ROS) in the regulation of intracellular Ca2+induced by angiotensin II ( Ang II) in the primarily cultured medullary neurons .METHODS:Primarily cultured me-dullary neurons were prepared from 14-day-old embryos of Sprague-Dawley rats in the study .The identification of medullary neurons was assessed by double-labeling immunofluorescence .To explore the role of ROS , mainly the superoxide ( O2 ·-) , the O2 ·-generation was measured using the fluorogenic probe dihydroethidium ( DHE) .To determine intracellular free cal-cium concentration ( [ Ca2+] i ) , the neurons were loaded with the Ca 2+-specific dye Fura-2/AM.The cell viability after adding Ang II was also examined using CCK-8 assay.RESULTS:Most of the cultured cells were medullary neurons , more than 80%of which were glutamate positive neurons .Ang II (5 μmol/L) increased the level of ROS within 10 min in the medullary neurons .Ang II at 5μmol/L induced a significant [ Ca2+] i increase in the medullary neurons , and the effect of Ang II occurred rapidly and reached a peak within 20 min after administration.The level of [Ca2+]i started to decline after washout .The Ca2+elevation induced by Ang II was significantly decreased by apocynin or TEMPOL .No significant differ-ence in the cell viability between control group and 5μmol/L Ang II treatment group was observed .CONCLUSION:ROS is involved in the regulation of [Ca2+]i induced by Ang II in the primarily cultured medullary neurons , suggesting a poten-tial intracellular signaling mechanism involved in the Ang II-mediated oxidant regulation of central neural control of blood pressure.

Results PCRanalysisofE2BandASPP2wasinconsistent with theoretical values;Particle numbers of virus were 5. 6 × 1012 VP/mL,infectious titer was 2 ×1011 IU/mL and specific activity was 3. 5%;ASPP2 protein expression could be detected when cells were infected with virus for 24 h;Growth inhibition of liver cancer cells could be found by adding recombinant ASPP 2 adenovirus;The level of RCA was less than 1 RCA/3. 0 ×1010 VP,in line with the standards of China Food and Drug Adminis-tration(SFDA).Conclusion Thequalitycontrolmethodswere established aiming at key characters of human recombinant AS-PP2 adenovirus,which may provide foundations for its quality standard and future applications.

RESUMO

This study was aimed to provide references for techniques in future application of traditional and natural drugs for tumor prevention and treatment. Domestic and foreign literatures on applications of gene chip technology in antitumor studies with natural and traditional medicine from 2000 to 2014 were reviewed. Corresponding database was established from aspects of published articles, drug intervention types, study fields, sample sources, chip technology platforms, repeatability of gene chip experiments, criteria of differential genes, and validation of gene chip experiments. Application experiences of gene chip in antitumor natural and traditional drugs were summarized. Shortcomings of gene chip technology application were analyzed deeply. The results showed that experimental gene screening was limited at the cellular level. More attentions should be paid to experiments at the animal and clinical levels. Scholars had paid more attention to expression level of mRNA and less attention to gene regulation level and epigenetic research of microRNA chip and DNA methylation chip. Gene chip experiments were lack of repeatability, which directly affected the evaluation results of difference gene and reduced reliability of gene screening. Screening results of genes should be verified not only at the mRNA level, but also increased at the protein level. It was concluded that gene chip was one of the most mature technologies to detect the level of gene expression, which was widely used in the research field of traditional and natural antitumor drug studies. Researchers should try to avoid deficiencies mentioned above in experiments related to gene chip technology.

<p><b>OBJECTIVE</b>To evaluate clinical treatment regularity of traditional Chinese medicine (TCM) on primary liver cancer and provide inspiration for the clinical use.</p><p><b>METHOD</b>Traditional Chinese medicine database on primary liver cancer was established to analysis the classification, frequency, dosage of TCM in clinical treatment.</p><p><b>RESULT</b>Tonic medicine is the most common medication, herbs for heat-clearing, promoting blood circulation for removing blood stasis, eliminating dampness and diuresis and regulating flow of Qi are more common medication, herbs for relieving exterior disorder and digesting are common medication; the first frequency of single herb is Atractglodis Macrocephalae Rhizoma, Poria, Codonopsis Radix. Popular classical prescriptions are Sijunzi Tang, Xiaochaihu Tang, YiguanJian, Xiangsha Liujunzi Tang, Xiaoyao Wan and Gexia Zhuyu Tang, Liuwei Dihuang Tang and Yinchenhao Tang et al. Gallic Gigerii Endothelium Corneum and Ophiopogonis Radix are most commonly drug for poor appetite. Astragali Radix is most commonly drug for fatigue. Corydalis Rhizoma, Toosendan Fructus are most common for liver pain; Pericarpium Arecae, Polyporus, Poria are most common herbs for ascites; Artemisiae Scopariae Herba is common drug for jaundice.</p><p><b>CONCLUSION</b>Replenishing qi to invigorate the spleen, sparsing liver to regulate the flow of vital energy, clearing heat and promoting diuresis, promoting blood circulation for removing blood stasis, nourishing yin and detoxification are the main principles for treating primary liver cancer. Improving clinical symptoms, signs and quality of life of patients with TCM is the key to clinical treatment.</p>

<p><b>OBJECTIVE</b>To in vitro compare the induction of extracts of Stellera chamaejasme ESC, ESC-1 and ESC-2 on NCI-H157 cell apoptotic.</p><p><b>METHOD</b>The apoptosis rate was inspected by flow cytometry; caspase-3, 8, 9 activities was measured by spectrophotometry. Fas, Fas-L, TNF-alpha, Trail-R, Cyto-C, Smac/diablo protein expressions of apoptosis pathway was observed by Elisa method.</p><p><b>RESULT</b>Compared with the control group, ESC, ESC-1, ESC-2 can significantly improve the apoptosis rate of NCI-H157 cell. ESC significantly improved cells caspase-3, 8 activity, ESC-2 can significantly improve the activity of caspase-3, 8, 9. ESC, ESC-1, ESC-2 significantly increased Fas expression and ESC significantly increased Fas/Fas-L ratio. ESC, ESC-1, ESC-2 significantly increased TNF-alpha protein expression. ESC-1 significantly lowered TRAIL-R expression. ESC, ESC-1, ESC-2 had no significant effect on Cyto-C. ESC-1, ESC-2 significantly reduced Smac protein expression.</p><p><b>CONCLUSION</b>The apoptotic effect induced by ESCs may be related to the regulation of death receptor pathway proteins. Induction mechanisms of ESCs were so complicated that it may have a two-way regulatory effect. Its induction in apoptosis is a result from comprehensive regulation and control.</p>

<p><b>OBJECTIVE</b>To study the scientific theory on detoxification (attenuation) of Stellera chamaejasme (ScL) by processing and the impact on drug effect of ScL before and after being processed with vinegar.</p><p><b>METHOD</b>The difference in ingredients of ScL before and after being processed vinegar was compared by using PHLC-MS technique. A subcutaneously transplanted tumor model of H22 hepatoma was established to compare the lethal effect and weight change between tumor-loaded mice and normal mice. After consecutive oral administration in tumor-loaded mice, the impacts on tumors and immune organs were compared before and after being processed with vinegar. Luciferase Report Gene was employed to investigate the target genes TGF-beta, AP1 and NF-kappaB.</p><p><b>RESULT</b>The LD50 (median fatal dose) of extract Zp1102 exhibited higher than that of the processed one Zp1103, that is 9. 89 g x kg(-1) vs. 16.85 g x kg(-1). According to the test, Zp1102 showed more effective anti-tumor activities in vivo than that of Zp1103 in a same dosage, with the tumor inhibitory rate 36.24% (P < 0.01) at the dosage of2 g x kg(-1) and 34.40% (P < 0.05) at the dosage of 1 g x kg(-1). At the dosage of 1 g x kg(-1), Zp1102 showed a tumor inhibitory rate of 34.52% (P < 0.05), much higher from 21.55% in Zp1103. Both Zp1102 and Zp1103 had basically no impact on the report gene NF-kappaB, besides that Zp1102 up-regulated the report gene after increase in NF-kappaB concentration and down-regulated TGF-beta, but Zp1103 can only up-regulate NF-kappaB expression without any impact on TGF-beta.</p><p><b>CONCLUSION</b>Processed ScL extracts show less toxic than unprocessed extracts and slight reduction in anti-tumor activity, which may be related to the regulation of transforming growth factor TGF-beta.</p>

<p><b>OBJECTIVE</b>To observe the effect of Tiangou Jiangya capsule (TJC) on blood pressure in renovascular hypertension rats and explore its possible mechanism.</p><p><b>METHOD</b>Seventy-two Wistar rats were randomly divided into normal control group, model group, captopril group, TJC small, medium and high dose groups. Non-invasive blood pressure measurement was used to detect the arterial blood pressure of rat tails. PRA, Ang II , ALD, 6-Keto-PGF1alpha, ET and TXB2 content in blood was measured by radioimmunoassay. NO content in blood was determined by method of nitrate reductase.</p><p><b>RESULT</b>The systolic, diastolic and mean pressure significantly increased, serum PRA, Ang II , ALD decreased, ET levels significantly increased in model group rats. TJC significantly reduced blood pressure, improved the plasma renin activity, decreased ET levels and increased NO content of model rats.</p><p><b>CONCLUSION</b>TJC can reduce blood pressure of renovascular hypertention rats, and the mechanism may be related to its regulating lower blood pressure regulation of the secretion of RAAS system and improving vascular endothelial function.</p>

<p><b>OBJECTIVE</b>To observe the anti-tumor effect of alcohol extract of Stellera chamaejasme-AESC, AESC-1 and AESC-2 in vitro.</p><p><b>METHOD</b>Inhibition rates of AESC, AESC-1, AESC-2 on five tumor cells (A549, NCI-H157, NCI-H460, BEL-7402 and SK-HEP-1 cell lines) were observed and IC50, GI50, TGI50 and LC50 were calculated with the methods of MTT and SRB.</p><p><b>RESULT</b>AESC, AESC-1, AESC-2 have inhibitory effect on all of 5 cell lines and have a certain time-dependent nature. On 72 hours, inhibition rates of 100, 200 mg x L(-1) of various extracts on each cell line (except A549) were between 64.82% and 92.27%. IC50 of AESC and AESC-2 on NCI-H460 and BEL-7402 cell lines is less than 20 mg x L(-1), IC50 of AESC-1 on SK-HEP-1 cell line is less than 20 mg x L(-1). The mean GI50, TGI and LC50 of AESC, AESC-1, AESC-2 on sensitive cell lines were less than 40, 90, 150 mg x L(-1) respectively.</p><p><b>CONCLUSION</b>AESC, AESC-1 and AESC-2 all have significant anti-tumor effect and have the different sensitivity to different cell lines; AESC-2 has the similar anti-tumor manner to the AESC and the anti-tumor active fraction of AESC may be mainly present in the AESC-2.</p>

<p><b>OBJECTIVE</b>To investigate whether extracts of Danshen and Chuanxinlian (SL) could promote the function recovery in pre-monocytic cell line THP-1 induced by TNF-alpha.</p><p><b>METHOD</b>SL extracts (0.125-2 g x L(-1)) were used to incubate THP-1 for 24 h before stimulation with TNF-alpha (20 microg x L(-1)), the adhesion, migration, lipid uptake and secretion of THP-1 were observed.</p><p><b>RESULT</b>SL (0.5-2 g x L(-1)) had obvious effect on decreasing the THP-1 adhesion. The number of passed membrane was much fewer than that of control cells in SL (0.125-2 g x L(-1)). SL (0.125-2 g x L(-1)) reduced the total cholesterol content significantly. The levels of IL-6 in SL (2 g x L(-1)) were significantly decreased,and IL-10 was increased than that before the treatment.</p><p><b>CONCLUSION</b>SL extracts could promote the function recovery such as adhesion, migration, lipid uptake and secretion of THP-1 induced by TNF-alpha, which probably is one of the mechanisms of inhibit the inflammatory reaction in initiation and development of AS.</p>

<p><b>OBJECTIVE</b>Wuji pill is a prescription of traditional Chinese medicine(TCM) and was composed of Rhizoma Coptidis, Fructus Evodiae Rutaecarpae and Radix Paeoniae Alba. The aim of this research is to investigate the effects of Wuji pill compound with different compatibility on the levels of enzymic activity of cytochrome P450 CYP3A1/3A2 in rat liver microsomes in vitro, and to confirm the compatibility mechanism of Wuji pill from the point of relationships between compound prescription of TCM and metabolism.</p><p><b>METHOD</b>With testosterone being a probe, the levels of enzymic activity of CYP3A1/3A2 were detected by HPLC, which were suppressed by Wuji pill with different compatibility in vitro.</p><p><b>RESULT</b>The IC50 of Rhizoma Coptidis, Fructus Evodiae Rutaecarpae, Radix Paeoniae Alba and 1"-9" of different level Wuji pill is: 38.96, 871.96, 15 519.17, 43.17, 60. 47, 276.12, 133.40, 118.08, 88. 47, 64. 36, 35. 13 and 39. 91 mg x L -', respectively. Rhizoma Coptidis and 1-9" of Wuji pill can suppress the enzymic activity of CYP3A1/3A2 significantly, and the capability of Rhizoma Coptidis in Wuji pill of action on CYP3A1/3A2 can be modified by different composition of Fructus Evodiae Rutaecarpae and Radix Paeoniae in Wuji pill, and there are statistical differences among the IC50 of 1#-9# of Wuji pill. While the ratio of Rhizoma Coptidis raises up in Wuji pill, Wuji pill may suppress the enzymic activity of CYP1A2 largely.</p><p><b>CONCLUSION</b>The reason why Wuji pill with different compatibility has different pharmacodynamics and pharmacokinetics characteristics is likely to lie in the difference of the capability of Wuji pill with different compatibility on CYP3A1/3A2. RESUMO

Objective To study the mechanism of Hezhong Jiangni Formnla(HJF) in protecting esophageal mucosa.Methods Rat models of reflux esophagitis were established by cardioplasty,pyloric ligation and roux-Y type gastrojejunostomy to evaluate the degree of esophageal mucosa injury.After treatment of HJF,the gastric juice was collected with the method of pyloric ligation to determine its volume and acidity and the activity of pepsin.The gastrointestinal motility was observed after oral use of charcoal powder.The level of malondialydehyde(MDA)in esophageal mucosa was measured to evaluate the esophageal antioxidative ability.Results HJF relieved the injury of esophageal mucosa dose-dependently;it had no obvious influence on the volume and acidity of gastric juice and the activity of pepsin,but it could obviously improve the gastrointestinal motility.MDA level of esophageal mucosa in the model group was increased significantly as compared with the control group.Pretreatment with HJF could greatly inhibited the formation of MDA.Conclusion HJF has protective effect on esophageal mucosa,which may be related to the improvement of gastrointestinal motility and its antioxidant action.

RESUMO

Objective To investigate the effect on proliferation and apoptosis by RNA interference to inhibit Nucleostemin(NS) gene expression in gastric carcinoma SGC-7901 cells.Methods The NS-siRNA expression vector was transfected into gastric-carcinoma cells with LipofectamineTM2000 reagent.Then we detected the cell proliferation inhibition ratio by MTT assay,the levels of NS gene expression in all groups by RT-PCR,cell cycle by flow cytometry,cell apoptosis ratio by Annexin-V-FITC/PI kit.Results Compared with that in the control group,cell proliferation in S group decreased;at 24,48 and 72 h the cell proliferation inhibition ratio was 53.21%,71.54% and 87.47%,respectively,the level of NS gene expression reduced in S group.G0/G1 phase cell was 58.34%,S phase cell was 20.68%,and the cell apoptosis was 26.85%.Conclusion RNA interference could substantially inhibit NS gene expression in gastric carcinoma SGC-7901 cells,decrease cell proliferation,arrest cell cycle and increase apoptosis ratio.
